As a Community Nurse, you will play a crucial role in delivering high-quality care to patients within their homes, promoting independence and improving overall health outcomes. Key Responsibilities: Conducting comprehensive assessments of patients' health needs and developing personalised care plans.Delivering a range of nursing interventions, including wound care, medication management, and symptom management.Providing emotional support and practical advice to patients and their families.Collaborating closely with multidisciplinary teams to ensure coordinated care delivery.Maintaining accurate and up-to-date patient records and documentation.Contributing to the evaluation and improvement of service delivery through feedback and reflection.
