A key contributor to the team function is to aid the individual, with the goal to leave the service and transfer back into the community. Responsibilities can include, implementing care packages, practicing and carrying out assessments under the legislation of adult social services framework and working with multiple agencies for the benefit of the individual.About youA successful candidate will have understanding of working within adult social services within various team such as, community, hospital, complex care, safeguarding.
