Nurse Case Manager - Liverpool My client is dedicated to delivering exceptional healthcare services to our community. We are looking for a dedicated and experienced Nurse Case Manager to join the team and contribute to improving patient care and outcomes. Key Responsibilities: Develop and implement individualized care plans for patients. Coordinate and monitor healthcare services to ensure effective and efficient care delivery. Educate and support patients and their families about treatment options and care plans. Collaborate with healthcare providers, social workers, and other professionals to optimize patient outcomes. Maintain accurate and up-to-date patient records and documentation. Qualifications: Registered Nurse (RN) with a valid license. Previous experience in case management or a similar clinical role. Strong communication, organizational, and problem-solving skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of healthcare regulations and best practices.
