This is a highly effective team within the service that works hard to ensure a smooth transitions for those individuals.About youYou will have a Social Work Degree/DipSW/CQSW within a minimum of 1 year post ASYE experience is required. You will be carrying out Menta Capacity assessments, Care Act 2014, and section 42 assessments.
