Your primary responsibility is ensuring the well:being and health of elderly residents or individuals with various medical conditions. Supervisions are a part of your role. Key Duties and Responsibilities * Work as part of an efficient team, contributing to the smooth running of the home.* Accurately assess the needs of residents and identify priorities of care.* Report any ill:health amongst residents and request GP/professional visits as necessary.* Ensure residents care plans are completed and maintained, involving residents, relatives, and other health care professionals.* Maintain accurate records and ensure each resident has an individualised care plan.* Ensure clear and concise handover reports at the end of each shift.* Assist in the delivery of care for residents with terminal or progressive illnesses.* Carry out regular checks on residents to ensure their health, safety, and well:being.* Ensure the provision of a high standard of nursing and social care.* Support new staff members in all aspects of their work, offering help and guidance where appropriate.* Act as a mentor to new and less experienced staff, providing relevant training and coaching.* Maintain good relationships and an empathetic, professional approach to residents and relatives.* Communicate effectively with residents, partners, stakeholders, and other relevant bodies.
