As an Adult Social Worker, you will be responsible for conducting individual assessments, facilitating self-assessments, and developing personalised care and support plans that maximise long-term independence and choice for adults.Key Responsibilities Conduct individual assessments and facilitate self-assessmentsDevelop personalised care and support plansConduct risk assessments and Mental Capacity Act assessmentsManage an allocated caseload in compliance with statutory requirementsLiaise with other local services and specialist teams to deliver a seamless serviceEncourage the contribution of adults and their advocates, relatives, carers, and support workersMaintain a thorough understanding of available local provision and make appropriate recommendations and referralsPromote the use of direct payments where appropriateProcure care and support including equipment and adaptations Requirements At least 4 years of experience as an Adult Social WorkerKnowledge of the Care Act 2024 and Mental Capacity legislationRegistered social work degreeSocial work England registeredAbility to manage an allocated caseload in compliance with statutory requirements and professional best practiceExcellent communication and interpersonal skillsAbility to establish and maintain effective working relationships with other local services and specialist teamsAbility to promote the use of direct payments where appropriateAbility to procure care and support including equipment and adaptations Apply NowIf you are an experienced Adult Social Worker looking for a new challenge, this company would love to hear from you.
