The role is a lively position and requires an individual who is willing to work in a challenging and rewarding environment.Key Responsibilities Conduct hospital discharge Triage/Duty, CHC MDT D2A reviews, and office-based assessments 2-3 times per week.Provide expert advice and support to service users, their families, and carers.Develop and maintain effective working relationships with colleagues, other professionals, and partner agencies.Ensure that service users receive high-quality care and support that is tailored to their individual needs.Participate in regular supervision, training, and development activities. Requirements Qualified Social Worker with a valid SWE registration.Minimum of 2 years of experience as a Social Worker in an Adult setting.Excellent communication and interpersonal skills.Ability to work independently and as part of a team.Flexibility to work outside of normal hours when required.
