Nurse Case Manager Position: Nurse Case Manager Location: Remote with travel to sites in NW Employment Type: Full-timeMy client is dedicated to delivering exceptional healthcare services to our community. We are looking for a dedicated and experienced Nurse Case Manager to join the team and contribute to improving patient care and outcomes. Key Responsibilities: To take joint ownership of a caseload of complex clients and manage their care package holistically and clinically To develop, review and implement client care plans and risk assessments in line with CQC regulations and best practice guidance. To co-ordinate and lead the transition (start-up) and ongoing continuity of all allocated complex care packages.. To engage in appropriate networking and to work proactively in partnership with Clinical Commissioning Groups and Social Services, attending case conferences/ MDT meetings as required. To ensure complaints are managed in line with internal organizational policies. Qualifications: Registered Nurse (RN) with a valid license. Previous experience in case management or a similar clinical role. Strong communication, organizational, and problem-solving skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of healthcare regulations and best practices. Knowledge of CQC regulations and quality assurance processes
