Case Manager - Manchester/NW Position: Case Manager Location: Manchester Employment Type: Full-timeMy client is a leader in providing high-quality healthcare services. They are seeking a skilled and compassionate Case Manager to join the team in the Manchester area. This role involves coordinating comprehensive care for patients, ensuring they receive the best possible outcomes. Key Responsibilities: To take joint ownership of a caseload of complex clients and manage their care package holistically in collaboration with the clients appointed Nurse Case Manager. To develop, review and implement client care plans and risk assessments in line with CQC regulations and best practice guidance. To co-ordinate and lead the transition (start-up) and ongoing continuity of all allocated complex care packages.. To engage in appropriate networking and to work proactively in partnership with Clinical Commissioning Groups and Social Services, attending case conferences/ MDT meetings as required. To ensure complaints are managed in line with internal organizational policies Qualifications: Valid license in the UK. Previous experience in case management or a related clinical role. Strong communication, organizational, and problem-solving skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of healthcare regulations and best practices. Knowledge of CQC regulations and quality assurance processes
