Community Nurse :Eastleigh/Winchester : Hampshire Nurse Seekers are proud to be working alongside an exceptional community healthcare provider who services their community with a focus on compassionate, patient:centred care. They are now looking to recruit a Nurse (RGN) to join their dynamic team and contribute to the care of their service users. Job Description: Manage a varied caseload, including patients with complex care needs. Deliver high:quality nursing care in the community, collaborating with patients, families, and healthcare professionals. Key Responsibilities: * Conduct health assessments and develop care plans.* Provide direct nursing care and manage chronic diseases.* Support patients with complex needs, including palliative and post:operative care.* Educate patients and families on health maintenance.* Coordinate with multidisciplinary teams.* Maintain accurate patient records.* Participate in community health initiatives. Qualifications: * Registered Nurse (RGN) with up to date PIN* Minimum 2 years of nursing experience, preferably in community or with Complex Care* Strong clinical, communication, and organisational skills.* Ability to work independently and in a team.* Valid drivers license and reliable transportation.
