The Ideal candidate needs to be experienced within the field but also someone who is highly driven and can identify positivity within a service user and have the ability to develop potential within.Key role and responsibilities for a Outreach Practitioner; Able to manage a small caseload of 14-15 clientsProvide Outreach SupportBuild a support NetworkDeliver One to One interventions Minimum requirements for a Outreach Practitioner; Experience of working within the Substance Misuse fieldAwareness of health issues relating to Substance MisuseExperience of Key Working and Care Planning Our client vision is for people of all ages to be safe, build ambitions for the future and to reach their full potential.
