This company is seeking a Complex Case ManagerComplex Case ManagerAbout the RoleThis company is seeking a Complex Case Manager to work in the NHS Continue Healthcare in Birmingham. The role is a temporary agency contract with a pay rate of £310-£320 per day paid via an umbrella.Responsibilities Undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needsAct as a Case Manager for individual clients within Birmingham Solihull ICB who are receiving continuing health care fundingFacilitate, plan and evaluate service provision to ensure the client achieves an optimum level of function and receives appropriate levels of care in the most suitable environmentWork jointly with health, social care and independent sector colleagues to ensure care management processes are appropriatePresent assessments to the BSOL continuing care panels for ratificationFollow the process laid out in the National Framework for NHS Continuing HealthcareBe responsible for complex clinical caseload comprising of assessments, undertaking of review, implementation on Personal Health Budgets and ongoing Case Management of Patients funded by Continuing Healthcare CHC, Mental Health & Learning DisabilitiesEnsure assessments conducted are robust, transparent and multi-agency where required for NHS Continuing Healthcare/NHS-funded Nursing CareAnalysis of supporting documentation to establish critical history of the patient’s relevant needs to produce a specialist nursing assessmentComplete new referrals for NHS Continuing Healthcare within 28 days as per the National Framework for NHS Continuing Healthcare Requirements Registered Nurse with a valid NMC PinExperience in Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needsExperience in Mental Health & Learning DisabilitiesAbility to work independently and as part of a teamExcellent communication and interpersonal skillsStrong analytical and problem-solving skillsAbility to manage a complex clinical caseloadKnowledge of the National Framework for NHS Continuing Healthcare Benefits Subsidised training / FREE mandatory trainingDedicated and experienced one-to-one consultant supportLoyalty reward scheme How to ApplyIf you are interested in this role, please send your CV to XXXX@servicecare.org.uk.Complex Case Manager - This company - Birmingham - Temporary Agency Contract - £310-£320 per day paid via an umbrella - Registered Nurse - Continuing Healthcare - Mental Health & Learning Disabilities
