The team is based on site at the hospital, the team works to discharge patients from hospital with the aim to create a care plan for them when they are due to go into community setting.About youA Social Work Degree/DipSW/CQSW within a minimum of 1 year post ASYE experience is required. The successful candidate will have previous experience of working with relevant legislation.
