Leaving Care WorkerLeaving Care WorkerJob DescriptionThe London Borough of Ealing are seeking a Leaving Care Worker to join their team. The successful candidate will be responsible for carrying out assessments of social care needs, including full financial assessments, within departmental guidelines. They will work with service users and their carers to agree on care plans that identify appropriate options. The Leaving Care Worker will also be responsible for initiating the provision of services based on agreed care plans and will record all information regarding needs that have not been met through assessment and bring it to the attention of the Team Manager.Key Accountabilities Responsible for an allocated workload, working to deadlines and prioritising work in line with departmental policy and procedure under the direction of a Team Manager or Deputy Team Manager.Participating in a duty rota and undertaking initial assessments of need, responding to emergencies, and answering inquiries from the public and other professionals about services.Undertaking the guidance of a Team Manager to be responsible for supervising contact visits by parents and other involved parties to children in care in a variety of settings.Monitoring Care plans and reviewing individual packages of care within the complete administration, using the appropriate information technology, to enable management to monitor progress of care plans and take action as required.
