About the Role:Position: Band 7 Clinical Nurse or ParamedicSpecialisation: Palliative Care Experience or Qualifications requiredSchedule: 7-day week, 8am to 6pmEmployment Type: Full-time and Part-time positions availableLocation: Kent Free on-site parkingStart Date: As soon as available, subject to standard checksRates: Competitive locum rates, either PAYE or Limited company Responsibilities Include:- Serving as the initial point of contact for new community referrals and telephone inquiries.- Participating in a rota of clinicians covering a 7-day service.- Providing advice and care to patients in clinic and home settings, as well as remotely via telephone and videoconferencing.- Focusing on individualized care, improving quality of life, and achieving patient-centered goals.- Working alongside other community teams and specialists to optimize patient experience and enhance team knowledge and development. To Succeed in This Role:- Proven community or in-patient palliative care experience at Band 6 or 7, or equivalent experience in areas such as Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology, or Oncology.- Motivated clinician with the ability to work independently and as part of a responsive and resilient team.- Experience in mentoring and supporting individuals and teams.- Advanced communication skills and excellent presentation skills both oral and written.
