The successful candidate will be responsible for the direct line management, supervision and approval of reablement plans for up to seven reablement staff, including SWs, Reablement Officers, and Social Care Officers. They will also be responsible for case direction and recording case directions, performance management, and reports to the reablement Team Management. The reablement Practice Manager will be part of the reablement management team and able to deputise for the TM if necessary.As the Reablement Practice Manager, you will attend Integrated Discharge Hub calls if required and have a good understanding of hospital to home D2A pathway.
