We are looking for an Adult's Social Worker to join the Adults Case Management Team.THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.About the teamThis team works with vulnerable adults over the age 18+, this team takes on cases with adults involved in the community setting. These individuals may have physical disabilities, mental health difficulties and or learning disabilities.We are looking for an Adult's Social Worker to join the Adults Case Management Team.THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.About the teamThis team works with vulnerable adults over the age 18+, this team takes on cases with adults involved in the community setting. These individuals may have physical disabilities, mental health difficulties and or learning disabilities. Key responsibilities are to carry out assessments, contribute to care packages and take part in the duty rota.About youThe successful candidate will have previous experience of working with relevant legislation such as Care Act 2014, MCA, Section 42. A Social Work Degree/DipSW/CQSW within a minimum of 1 year post ASYE experience is required.
