They help patients to be as independent as possible after being discharged from hospital to prevent them from moving into residential or nursing homes. About youThe ideal candidate will have post-qualifying experience in Adult Social Work. Experience within Hospital or Intermediate Care lends well to this position. A degree in Social Work Degree/DipSW/CQSW. Have a Practice Educator qualification.
