We are looking for a Social Worker to join a Hospital Integrated Discharge Team.We are looking for a Social Worker to join a Hospital Integrated Discharge Team. THIS ROLE REQUIRES A SOCIAL WORK QUALIFICATION WITH A MINIMUM OF 2 YEARS POST QUALIFIED EXPERIENCE. About the teamThis is a fast paced team who work to ensure patients are discharged from hospital as safely as possibly – with all necessary resources. Use of D2A procedures, completing mental capacity act assessment and dealing with safeguarding procedures are all day to day responsibilities in this role. The team prioritises working with service users and their carers promote and maintain their dignity, choice and independence. About youA degree within Social Work Degree/DipSW/CQSW with a minimum of two years experience working in an Adult’s Social Work setting are essential for this position. Experience in a hospital or community based team is experience that lends well to this team.
