Personal Advisor - Leaving Care Team - HorshamPersonal Advisor - Leaving Care Team - Horsham About the CompanyWest Sussex County Council and provides direct support to over 600 young people each year. The company supports, and plays a key role in, the journey of young people between the ages of 16 to 21, and up to 25 in some cases, as they leave care and transition towards an independent future as young adults.About the RoleThe company is seeking a Personal Advisor to join their team in Horsham. As a Personal Advisor, you will work with directly with individual young people who are leaving the care of the Local Authority by helping to assess and meet their learning, personal and social needs. You will ensure they are supported safely and appropriately which is essential in understanding and managing any risks that may be presented and leading on any safety planning that is required.Responsibilities Assess and meet the learning, personal and social needs of young people leaving careEnsure young people are supported safely and appropriatelyUnderstand and manage any risks that may be presentedLead on any safety planning that is requiredDevelop the skills needed for young people to live independentlyBe aspirational in identifying and achieving their goalsSupport young people in healthy living, stable homes and good educational opportunitiesWork with care leavers between the ages of 17-25 in their transition to adulthoodSupport care leavers with complex needs including mental health, substance use and offending behaviours Requirements Experience of working with children as a Personal Advisor or in Children's Social CareAmbitious for young people, committed to improving services for them and passionate about making a real difference to their livesExcellent communication and interpersonal skillsAbility to work independently and as part of a teamAbility to manage a complex caseload and prioritise workload effectivelyFull-time availability and willingness to work in a hybrid environment Apply NowIf you are passionate about supporting young people and have the required experience and skills, this is an excellent opportunity for you.
