This team supports children and young people who have complex needs associated with their disability and work closely with adult services colleagues to ensure a smooth and seamless transition into adulthood.The role will include ongoing case management and risk monitoring undertaking of a variety of assessments and the implementation and review of care and support plans.About the roleYou will need a Social Work degree to be considered for this position, and previous experience within a Children’s Social Work team is essential. You might also have experience with Children in Need, Child Protection, or Safeguarding.What’s on offer Salary up to £38,223 Depending on experienceGenerous annual leave starting at 27 days.Training and career development opportunitiesHybrid working opportunities.
